Description

INTENT TO SOLE SOURCE    The Department of Veterans Affairs, Network Contracting Office 4 (NCO4) intends to sole source with COSMED USA, LLC to provide a service contract for preventative maintenance and repair for existing COSMED Q-NRG+ machines at the Michael J, Crescenz VA Medical Center.  The objective of this sole source award is for a Firm-fixed Price Contract, consisting of a base year plus four option years, with COSMED USA, LLC, 1850 Bates Avenue Concord, CA 94520, Unique Entity ID 020221102. This sole source determination is in lAW RFO 6.103-1(b) and RFO 12.204(a) utilizing simplified procedures.  The North American Industry Classification System (NAICS) code for this procurement is: 811210, Irradiation Apparatus Manufacturing. The Small Business Administration (SBA) size standard is: $34.0 Million. The PSC code is: J065, Maintenance, Repair, and Rebuilding of Equipment Medical, Dental, and Veterinary Equipment and Supplies. This contract will be exempt from wage determination.  The notice of intent to sole source is not a request for quotes. Interested vendors that can provide this requested service must respond no later than 10/19/2026 by 10:00 AM EST. Interested vendors must send a capability statement, no more than one page in length; to include Company Name, SAM UEI Number, Socioeconomic Category, brief description of your business entities capability to provide these services.  All information shall be provided at no cost or obligation to the Government. Offeror must provide the above requested information.  A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.                 Statement Of Work    Contractor shall develop and provide a comprehensive preventive maintenance service plan and furnish all skilled labor provided by technician certified to work on the Cosmed Q-NRG+ equipment. The Extended Service Plan, ESP4-Q-NRG+ shall include annual preventative maintenance including product certification, replacement of consumables, software/firmware update, priority scheduling and 2-day return freight for the Cosmed Q-NRG+ located at the Corporal Michael J, Crescenz VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA 19104.     SPECIFIC CONTRACT WORK REQUIREMENTS  Preventive Maintenance Inspections (PMI)  Includes:  Annual Preventative maintenance  Product Certification  Replacement of consumables  Software/Firmware Update (not including major version upgrades(i.e. 1.X to 2.X, 2.X to 3.X or Network/HL7 Upgrades)  Priority scheduling  2-day Return Freight    Location and Work Schedule  Corporal Michael J.
