INTENTÂ TO SOLE SOURCEÂ

The Department of Veterans Affairs, Network Contracting Office 4 (NCO4) intends toÂ soleÂ sourceÂ with COSMED USA, LLC to provide a service contract for preventative maintenance andÂ repair forÂ existing COSMED Q-NRG+ machines at the Michael J,Â CrescenzÂ VA Medical Center.Â 
TheÂ objectiveÂ of this sole source award is for a Firm-fixed Price Contract, consisting of a base year plus four option years, with COSMED USA, LLC, 1850 Bates Avenue Concord, CA 94520, Unique Entity ID 020221102. This sole source determination is inÂ lAWÂ RFO 6.103-1(b) and RFO 12.204(a)Â utilizingÂ simplified procedures.Â 
The North American Industry Classification System (NAICS) code for this procurement is: 811210, Irradiation Apparatus Manufacturing. The Small Business Administration (SBA) size standard is: $34.0 Million.
