The Virginia Army National Guard is seeking on-demand occupational health medical surveillance services for employees who may face workplace health hazards. - Government buyer: Department of the Army, Virginia Army National Guard (VAARNG), USPFO Activity VA ARNG. - Requested services: - Provide an all-inclusive mobile clinic and qualified medical personnel, equipment, tools, and materials at designated Virginia locations; the Government provides workspace. - Perform phlebotomy and laboratory testing, including chemistry panels, CBC, urinalysis, and selected blood lead/ZPP testing; vision and visual acuity examinations; pulmonary screening with respiratory clearance documentation; and hearing screening/audiograms. - Provide related medical administration and job-related surveillance for full-time and temporary employees who are or could be exposed to workplace hazards. - Volumes and part numbers: No fixed examination or testing quantities and no product part numbers are specified; volumes depend on employee needs and individual orders. - Notable requirements: Use appropriately qualified and licensed staff, including occupational medicine physicians or certified pulmonary function technologists, medical technicians, and CAOHC-certified audiogram screening technicians. Provide two hard copies of exam documentation within 30 business days after each event and an event summary report within 3 business days. The Government provides at least 20 calendar days' notice before an event. - OEMs and vendors: No commercial OEMs or vendors are named in the provided materials. - Maximum contract ceiling: $350,000.

Description

The Virginia National Guard is looking for a vendor to provide medical exams to employees who may have been exposed to hazard materials.

1. COMMUNICATION WITH THE GOVERNMENT:

A. Questions: Submit written questions via email ONLY to the contract specialist Katina Moore at katina.l.moore7. civ@army.mil. No phone calls will be accepted. Questions will be received until October 9, 2026, no later than 11:00 a.m. Eastern Standard Time (EST).

2. ADDENDUM to 52.212-1, Instructions to Offerors -Commercial Items (SEP 2023)  

A. Addendum to FAR 52.212-2(b), Submission of offers. The offeror shall submit an electronic copy of their dated quote via e-mail to: katina.l.moore7.civ@army.mil with the solicitation number W912LQ-27-A001 in the subject block by October 19, 2026, no later than 11:00 a.m. EST.  

Facsimile quotes will not be accepted. Offerors who do not provide all the required information with their quotes may be deemed NONRESPONSIVE for further consideration for the solicitation and award. In addition to the items required by 52.212-1(a), the offeror shall complete and provide the following with their submission:

1. Signed and completed Information Sheet. (Attachment 1) 2. Completed Reps and Certs for 52.219-1 3. Proof of medical liability insurance 4. Pricing Schedule attached.
