Pierce County Human Services Aging and Disability Resources (ADR) is seeking qualified organizations to deliver Health-Related Social Needs (HRSN) Nutrition Support Services for Medicaid recipients not currently receiving in-home long-term care support services (Non-LTSS Clients). - Government Buyer: - Pierce County Human Services - Aging and Disability Resources (ADR) - Products/Services Requested: - Medically Tailored Meals (MTMs) - Up to three meals per day, tailored to individual health conditions - Requires certification and assessment by licensed health professionals (Registered Dietitians or Primary Care Providers) - Nutritional assessments and care planning included - Fruit and Vegetable Provisions - Provision or purchase of fruits and vegetables from participating retailers and farms - Authorized for daily, weekly, or monthly delivery - Includes replacement of spoiled items and documentation of deliveries - Unique/Notable Requirements: - Only organizations with current contracts with Washington State DSHS Home and Community Living Administration (HCLA) and/or Pierce County ADR are eligible - Must serve the entire county - Services must comply with evidence-based nutrition and food safety guidelines - Person-centered and culturally appropriate service delivery - Documentation required in the CLC-GetCare database - Reimbursement rates match negotiated LTSS rates - Open and continuous solicitation; applications accepted at any time - No specific OEMs or commercial vendors are named; eligibility is restricted to organizations with existing contracts for these services.
Description
Pierce County Human Services Aging and Disability Resources is seeking qualified organizations to provide nutrition support services under the Health-Related Social Needs (HRSN) Program for eligible Medicaid recipients not currently receiving in-home long-term care support services. The services include Medically Tailored Meals and/or Fruit and Vegetable Provisions, which must be provided across the entire Pierce County area. Organizations must already have contracts with DSHS HCLA and/or PCHS ADR for these services to be eligible. The RFQ is open and continuous, allowing applications at any time, with the intent to qualify organizations for contract negotiation opportunities.
