The Wisconsin Department of Health Services, Division of Quality Services, seeks proposals for surveyor services to initiate complaint investigations and recertifications for Long-Term Care and Non-Long-Term Care facilities. Long-Term Care facilities include Intermediate Care for Individuals with Intellectual Disabilities facilities, and Non-Long-Term Care facilities include hospitals and End Stage Renal Disease facilities. The solicitation has two lots, and vendors may submit proposals for either or both; proposals must identify the lot number. The process may result in up to three separate contracts for each lot.
