HRSA is seeking public comment on revisions to Health Center Program information-collection forms, not procuring products or services. - Government buyer and offices: Health Resources and Services Administration (HRSA), within the U.S. Department of Health and Human Services (HHS); the revised Information Collection Request was submitted to the Office of Management and Budget (OMB) for review. - Proposed form changes: Updates to eight existing forms: Form 5A: Services Provided; Form 5B: Sites; the Form 5A and Form 5B scope-adjustment checklists; the checklist for deleting a service site; the checklist for adding a service site; the checklist for adding a service; and the checklist for deleting a service. - New forms: Checklist for Replacing a Service Site in Scope; Quality Improvement Fund (QIF) Final Report; and Loan Guarantee Progress Report. - Purpose and users: Forms support applications and renewals, progress reporting, program monitoring, and scope-of-project changes for Health Center Program recipients and look-alikes. HRSA says the revisions are intended to improve scope documentation and oversight, streamline scope-change processes, and improve the accuracy of requests. - Estimated paperwork impact: HRSA estimates the revisions will reduce annual reporting burden by 1,621 hours. - Procurement status: This is a public-comment notice, not a procurement or award. No products, services, quantities, part numbers, OEMs, or vendors are identified.
