HHS is seeking public comment on proposed revisions to reporting forms for the Unaccompanied Refugee Minors Program, not soliciting products or contracted services. - Government organizations: The Office of Refugee Resettlement (ORR), within the Administration for Children and Families (ACF), U.S. Department of Health and Human Services (HHS). - Forms under review: ORR-3 covers initial placement, termination, reentry, and case changes; ORR-4 records services received and outcomes. The forms are identified by OMB control number 0970-0034. - Proposed changes: Consolidate follow-up outcome reporting into one termination report; have providers report youth outcome measures instead of youths self-reporting; eliminate selected report types and unnecessary questions; and simplify questions, response options, and instructions. - Expected effect: The revisions are intended to reduce reporting burden by approximately 41 percent. Respondents are URM state or replacement designee agencies and URM provider agencies. - Procurement status: This is a request for public comment, not a procurement opportunity. No OEMs, vendors, products, quantities, or contracted services are identified.

Description

The Office of Refugee Resettlement is requesting a three-year extension with revisions of the Unaccompanied Refugee Minors Program ORR-3 and ORR-4 reports. The ORR-3 report records initial placement, termination, reentry, and changes in a youth’s case; the ORR-4 report records services received and outcomes. Proposed revisions streamline reporting, replace multiple follow-up outcome reports with one termination report, remove some questions and report types, and shift youth outcome reporting to providers. Comments are due November 16, 2026.
