The Department of the Navy is seeking a long-term lease of medical and office space in Crestview, Florida, for Defense Health Agency providers. - Government buyer: Department of the Navy, Naval Facilities Engineering Systems Command Southeast (NAVFAC Southeast), Real Estate Contracting Office (RECO). The space supports the Defense Health Agency (DHA); NAVFAC Real Estate and DHA facilities offices are also referenced in the lease rider. - Requirement: Lease approximately 12,000 rentable square feet of office/medical space in the Crestview area near Eglin Air Force Base, within ZIP Code 32536 or 32539. The space may be in more than one premises. - Minimum facility requirements include climate control, rooms of at least 120 square feet, 10-foot ceilings, and 117 parking spaces, including five accessible spaces. - The space must be ready for occupancy before the stated occupancy deadline. - Lease and facility responsibilities: The lessor maintains the building, leased space, roof, structural components, major systems, fixtures, landscaping, and parking; provides required security and access arrangements; and pays property taxes and property/hazard insurance. The Government separately obtains and pays for electric, water, sewer, and telephone service. - Term: One-year firm initial term with up to 19 additional one-year renewal options, for a maximum potential term of 20 years. Renewal options are at the Government’s discretion. Attachment materials differ on the Government’s termination notice period, stating 60 or 90 days after the initial term. - Offer evaluation and supporting requirements: Offers are evaluated on a best-value trade-off involving location, facility characteristics, and price. A prelease fire protection and life safety evaluation is required using GSA Form 12000; space on or above the sixth floor requires evaluation by a qualified professional fire protection engineer. - OEMs and vendors: No commercial OEMs, vendors, or specific products are named. GSA is identified in connection with the fire safety evaluation form, not as a product vendor.
