South Dakota is seeking partner agencies to staff and support Bright Start, its statewide home-visiting program. - Government buyer: South Dakota Department of Health, Division of Family and Community Health, Office of Home Visiting (OHV); the RFP contact is Carrie Churchill. - Program model and named organizations: Bright Start currently uses the Nurse-Family Partnership (NFP) model. NFP is a service model, not an identified vendor or OEM; no vendors or manufacturers are named. - Requested staffing and services: - Sioux Falls area: one Team Lead and five to seven Nurse Home Visitors for Minnehaha and Lincoln counties; an optional Community Health Worker/Outreach Assistant may be proposed. - Other statewide locations: up to 10 Nurse Home Visitors; applicants may propose some or all positions and must identify the locations. - Two Program Assistants/Referral Specialists and a CQI Specialist estimated at 0.2 FTE. - Agencies will employ, supervise, train, and support staff; deliver voluntary home visits, referrals, outreach, intake coordination, program administration, data and documentation, and continuous quality improvement (CQI). - Additional budgeted service: The budget template includes a $50,000 marketing contract for public education and outreach, including print materials, radio, and social media. This is a budget line, not a stated quantity of services. - Notable qualifications and requirements: Nurse Team Leads need a current RN license and an accredited nursing degree at the BSN level or higher; Nurse Home Visitors need a current RN license and an accredited nursing degree at the associate level or higher. The CQI role calls for CQI experience and relevant public health, maternal and child health, early childhood, or home-visiting experience. CHW certificate-level training is strongly desired for relevant outreach and assistant roles, or may be required within one year of hire. - Program and operating requirements: Agencies must support required NFP, Bright Start, confidentiality, and HIPAA training; provide supervision and operational support; comply with program, data, confidentiality, and fiscal requirements; and protect state-owned program and client records. Agencies must be able to adapt if OHV transitions to another federally compliant, evidence-based home-visiting model.
Description
The South Dakota Department of Health, Office of Home Visiting, seeks proposals from partner agencies to support staffing and delivery of Bright Start home visiting services. Selected agencies will employ, supervise, and oversee program personnel, including nurse home visitors, a team lead, a community health worker, program assistants/referral specialists, and a continuous quality improvement specialist. The program currently delivers the Nurse-Family Partnership model to eligible pregnant women across South Dakota using evidence-based home visiting practices. The State may transition to a different evidence-based model if program, funding, or federal guidance requires it, and asks agencies to indicate their capacity to adapt.
