Description

The Transition to Adulthood Program is designed to help young adults overcome barriers, build independence, strengthen well-being, and prepare for a successful transition to adulthood. The Qualified Service Provider will deliver individualized mentorship, coaching, advocacy, and life-skill development using a relationship-based and trauma-informed approach.
