Description
HRSA submitted an Information Collection Request for the Healthy Start Evaluation and Quality Improvement program to OMB for review and approval under the Paperwork Reduction Act. The request would reinstate four participant-level data collection forms, with minor changes to clarify instructions, update selected response options, and capture the month of infant birth and/or death. The data collection supports monitoring and evaluation of Healthy Start grantee and participant progress and program outcomes. Comments were due by November 12, 2026.
