Description

Central Texas Community Health Centers d/b/a CommUnityCare is seeking proposals for referral management software for its patients. The solution should track referrals to specialists across Travis County and surrounding areas from creation through final resolution, including scheduling, completion, receipt of consultation, and communication back to the originating provider. The anticipated agreement term is three years with one optional renewal period. The stated proposed budget is $150,000.
