B.2 SCOPE OF WORK B.2.1 GENERAL This firm fixed price requirement is for the annual preventative maintenance of two Canon/Toshiba CT machines, model: Aquilion Prime 160, serial numbers FLB16Y2138 and FLB1742181 located at the Buffalo VA Medical Center 3495 Bailey Avenue Buffalo NY 14215, from 11/01/2027 to 10/31/2027 to include four (4) option years.

B.2.2 SCHEDULE 1. All work shall take place during normal business hours, Monday - Friday, 8:00am-5:00pm, excluding the following federal holidays. a. New Year s Day b. Birthday of Martin Luther King Jr. c. Presidents Day d. Memorial Day e. Juneteenth f. Independence Day g. Labor Day h. Columbus Day i. Veterans Day j. Thanksgiving Day k. Christmas Day l. Any other day deemed a Holiday by the President

B.2.4 ANNUAL PREVENTATIVE MAINTENACE OF CANON/ TOBISHA CT MACHINES The contractor shall ensure the CT machines are properly maintained and repaired in a timely manner.

The contractor shall perform preventative maintenance service four times a year for the two (2) Aquilion Prime 160 CT machines at the Buffalo VA Medical Center.

The contractor shall service and provide replacement of X-ray tubes.

The contractor shall provide new, standard parts that comply with applicable performance and reliability specifications.

The contractor shall provide parts that shall be warranted to be free from defects in material and workmanship for a period of twelve (12) months from the date of installation.

The Contractor warrants that all repair services shall be performed according to current manufacturer s specifications for such equipment.

The Contractor warrants that the labor and services shall be performed in a good and workmanlike manner and shall comply with all applicable laws and regulations.

The contractor shall provide a firewall between hospital network and CT machines.

B.2.5 RESPONSE TIME

Onsite Response The contractor shall provide the Field Service Engineer (FSE) on-site response time of 4-6 hours. and 30-minute telephone response time for corrective maintenance repairs.

Telephone Response The contractor shall provide the Field Service Engineer (FSE) a telephone response time within 30 minutes for corrective maintenance repairs.

B.2.6 PATIENT PRIVACY

The Contractor shall abide by the rules and regulations that relate to the privacy, security and confidentiality of patient information and patient-identifiable information.

B.2.7 GENERAL LIABILITY

The Contractor shall maintain at its sole cost and expenses, commercial general liability insurance for bodily injury and property damage liability.

B.2.8 SERVICE DOCUMENTATION

Upon completion of the repair, the contractor shall provide an electronic copy of the field service report (FSP) to the COR and applicable VA HTM technician. Additional Links:Department of Veterans Affairs
