Description
This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to fulfil a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method.
The Great Lakes Acquisition Center (GLAC) is soliciting businesses for SLIT LAMP installation for the Captain James A. Lovell Federal Health Care Center (FHCC) located at 3001 Green Bay Rd., North Chicago, IL 60064.
Any company submitting a quote must include a letter and/or certification from Haag-Streit/Reliance stating they are trained and authorized by Haag-Streit/Reliance to install Haag-Streit and Reliance equipment.
Responses should include the following: (a) a letter or certification from Haag-Streit/Reliance demonstrating training and authorization by Haag-Streit/Reliance, (b) socio-economic status, (c) SAM UEI number, (d) a capabilities statement, and (e) a point of contact number.
