Funding depends on application review; these figures do not establish a total contract value. - Named organizations: Community Health Programs, Community Health Center of Franklin County, Outer Cape Health Services, and Island Health are cited as examples of rural-located FQHCs, not as suppliers. Adobe is mentioned only as an optional application-form reader, not as a supplier. - Notable requirements: Selected participants have fiscal and performance reporting obligations.
