Description
The Administration for Children and Families’ Office of Refugee Resettlement requests a three-year extension with changes to the Mental Health Assessment Form and Onsite Health Intervention Form. Proposed changes combine the two Public Health Investigation Forms into a single Onsite Health Intervention Form and revise the Mental Health Assessment Form. The revisions are intended to improve transparency, reduce burden, improve data quality, and align the forms with ORR requirements. The request also includes sharing relevant health information with the Department of Homeland Security in specific circumstances to support continuity of care. Public comments are due November 4, 2026.
