The State of South Dakota Department of Social Services (DSS), Division of Medical Services (DMS), is seeking proposals for a Case Management Tool to support the Medicaid Primary Accountable Care Transformation (PACT) initiative. - Government Buyer: - State of South Dakota Department of Social Services, Office of Procurement Management - Products/Services Requested: - Case Management Tool (software solution) to support care coordinators, case managers, and clinical teams - Implementation, integration, training, maintenance, and operations services for the tool - Key Technical and Functional Requirements: - Must support approximately 20,000 Medicaid members, including rural, Tribal, and justice-involved youth populations - Interoperability with South Dakota Health Link (SDHL) and Health Information Exchange (HIE) - Compliance with HL7 FHIR R5 and current/future CMS interoperability standards - 10-year data retention for member records - Flexible hosting options: preference for State Azure PaaS, but vendor-hosted solutions are acceptable (with additional security validation) - U.S.-based development required - Scalable to accommodate changes in program participation and user volumes (about 100 users at launch, up to 60 carceral settings for referrals) - Comprehensive user training and ongoing support - Unique/Notable Requirements: - No existing case management system to replace; this is a new implementation - Must be able to operate independently if SDHL is unavailable - Phased and iterative deployment with a post-go-live stabilization period - Ongoing Maintenance and Operations (M&O) for at least two years, with optional one-year renewals - No specific OEMs or vendors are named in the solicitation.

Description

The State of South Dakota Department of Social Services is seeking proposals for a Case Management Tool as part of the Rural Health Transformation initiative. The tool will support care coordinators, case managers, and clinical teams in delivering coordinated, person-centered services for Medicaid members, including juveniles under section 5121, CAA 2023. The goal is to promote shared accountability for cost, quality, and health outcomes among Medicaid enrolled primary care clinics, Rural Health Clinics, Federally Qualified Health Centers, and Tribal partners, and to build a resilient, connected, and sustainable rural health system.
