The Indiana Department of Health is seeking a vendor to supply and deliver metabolic formula products for individuals with inborn errors of metabolism under the Metabolic Formula Program. - Government Buyer: - Indiana Department of Health - OEMs and Vendors: - Abbott - Ajinomoto Cambrooke - Mead Johnson - Nutricia - Vitaflo - Products/Services Requested: - Wide range of metabolic formula products (powders, drinks, tablets) for various metabolic disorders - Specific part numbers and quantities for each formula (e.g., Calcilo XD, Cyclinex-1, GLYTACTIN BetterMilk, PhenylAde, PKU Sphere, Tylactin, Anamix, Lophlex, Periflex, Maxamum, Duocal, Polycal, Monogen, FruitiVits, Glycosade, Single Dose AA, etc.) - Quantities range from 1 to 60 units per item, with some items (e.g., PhenylAde PheBLOC Tablets) requested in much larger quantities - Unique or Notable Requirements: - Vendor must provide MMCAP pricing for formulas from the listed OEMs - Delivery required directly to recipients' residences within one business day of order placement - Monthly KPI reporting and secure/confidential data handling - 3% Indiana Veteran Owned Small Business (IVOSB) subcontracting goal - Contract term is five years with an optional one-year renewal - Formulas must cover a broad spectrum of metabolic conditions (PKU, MSUD, Homocystinuria, Tyrosinemia, Glutaric aciduria, Urea cycle disorders, etc.)
