REQUEST FOR INFORMATION / SOURCES SOUGHT for Patient Ceiling Lift DEPARTMENT OF VETERANS AFFAIRS, VISN 23 Source Sought: The VA is seeking business sources qualified to provide some, or all, of the items listed below. This is NOT a solicitation.

POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE:

1. Company name, address, phone number, primary contact(s), e-mail address.

2. State if your firm distributes the Brand Name items, or Equal items.

3. SAM UEI number.

4. NAICS (North American Industry Classification System) code 339113 Surgical Appliance and Supplies Manufacturing.

5. State if your firm is a large business, small business, service-disabled-veteran-owned small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small disadvantaged business.

   1. State if your firm has the required items on Schedule (FSS, GSA, SEWP, other ).

6. Provide a letter from the manufacturer verifying your firm is an authorized distributor.

   1. Provide the country of origin for each of the required items.

REQUIRED ITEMS:

The government is seeking information from capable sources that can provide a ceiling-mounted bariatric patient lift system meeting the following general requirements:

Ceiling-mount patient lift/hoist system Minimum lifting capacity of approximately 800 lbs Rechargeable/battery-powered operation Handheld and/or remote controls Smooth, controlled lifting and lowering Emergency stop and emergency lowering safety features Quiet operation suitable for a clinical environment Ceiling track system Capable of supporting bariatric patient transfers System capable of modification or expansion, as applicable Maintenance/service notification or tracking capability Compliant with applicable patient-lift and medical equipment safety standards Includes all necessary equipment

The Government s anticipated requirement is for a Guldmann GH3+ bariatric ceiling lift system or equal. Respondents offering an equal product should identify the manufacturer and model proposed and provide product literature demonstrating the product s capabilities.
