The Mercer County Department of Human Services, Office on Aging/Aging and Disability Resource Connection (ADRC), is soliciting proposals to provide County Direct Funding of $530,496.00 per year. The contract term options include a two-year contract with a one-year extension, or a one-year contract with up to two one-year extensions. The funding supports services and programs for Mercer County older adults aged 60 and older and residents with physical or developmental disabilities. Priority is given to individuals with the greatest social and economic needs, minorities, those at risk of institutional placement, older individuals with Alzheimer's disease and related disorders, caregivers, and residents with severe disabilities.
