The City of Cleveland EMS is seeking uniform items and related services for its captains: - Government Buyer: - City of Cleveland, Division of Purchases & Supplies, EMS Department - OEMs and Vendors: - Flying Cross (shirts) - 5.11 Tactical (pants and boots) - Vendors must be authorized distributors for both brands; manufacturer authorization letters are required - Products Requested: - Flying Cross Men's Long Sleeve Shirt (White, #35W5400): 3 units - Includes two custom Cleveland EMS shoulder patches per shirt - Flying Cross Men's Short Sleeve Shirt (White, #85R5400): 6 units - Includes two custom Cleveland EMS shoulder patches per shirt - 5.11 TAC LITE EMS Pant (Dark Navy, #74363ABR): 6 units - 5.11 ATAC 2.0 8" Tactical Boot (Black, #12391ABR): 3 units - Black Garrison Belt with Silver Buckle: 3 units - Services Requested: - Design, provide, and sew two custom Cleveland EMS shoulder patches per shirt (total: 18 patches) - Patches must match current captain color, font, graphics, and black border; subject to approval by EMS Commissioner - Unique/Notable Requirements: - No substitutions allowed for specified items - Vendors must be available within two days of award for captain measurements - Delivery required within three weeks after measurements - Quality and fit subject to inspection and approval by EMS officials - All items delivered to Cleveland EMS Headquarters - Strict adherence to brand, custom patch design and sewing, rapid vendor response for fittings, and mandatory manufacturer authorization letters
