Pierce County Human Services, through its Aging and Disability Resources (ADR) division, is seeking qualified organizations to provide Health-Related Social Needs (HRSN) Nutrition Support Services under Washington’s 1115 Medicaid Demonstration waiver. - Government Buyer: - Pierce County Human Services, Aging and Disability Resources (ADR) - Services Requested: - Medically Tailored Meals (MTMs): Preparation and delivery of meals tailored to medical needs for eligible Medicaid recipients (Non-LTSS Clients) in Pierce County - Fruit and Vegetable Provisions: Provision and delivery of fruits and vegetables to eligible Medicaid recipients (Non-LTSS Clients) in Pierce County - Eligibility Requirements: - Only organizations with an existing contract with Washington State DSHS Home and Community Living Administration (HCLA) and/or Pierce County Human Services ADR for serving LTSS Clients are eligible - Providers must be able to serve the entire geographic area of Pierce County - Compliance with program documentation and policies is mandatory - No specific OEMs or commercial vendors are named, as this is a qualifications-based solicitation for service providers - Notable Requirements: - County-wide service coverage - Mandatory eligibility criteria for applicants - Services must be delivered to Medicaid recipients not currently receiving in-home long-term care support services (Non-LTSS Clients)
Description
This solicitation is for Health-Related Social Needs (HRSN) Nutrition Support Services to be provided in Pierce County, Washington. The services require organizations to have an active DSHS contract and to serve the entire Pierce County area.
