Combined Synopsis/Solicitation for Commercial Items This is a combined synopsis/solicitation for the acquisition of commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this announcement. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation Number: RFQ-26-IHS1528504-01 This solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular (FAC) 2026-01, Effective 3/13/2026. The Contractor will deliver services over standard circuit-switched systems and digital circuit services for Kayenta Health Center, Inscription House Health Center, and Dennehotso Health Station. This includes minor hardware and software changes/installation to support the changing business role of Kayenta Service Unit, minor maintenance to hardware supporting telecommunication services, and ongoing system support. Section B – Products or Services and Prices/Costs B.1 Brief Description of Services The telecommunication services aim to maintain continuity of critical and timely services between Kayenta Service Unit, all NAIHS facilities, non-IHS facilities, patients, and Area Office partners. The primary goal is reliable, continuous telecommunications connectivity supporting healthcare delivery and business functions, including: Supporting healthcare service availability and accessibility Supporting healthcare quality, safety, cost, and value objectives Supporting insurance, long-term care, workforce, and administrative functions Supporting technological advances and innovation The Division of Information Technology aligns its operations to support the Navajo Area Indian Health Service Strategic plan, Mission, and Business Objectives by: Renewing and continuing the delivery of IT services and systems via telecommunication services Supporting IT and Telecommunication technologies under the Federal Enterprise Architecture, GSA Services, and Verizon networks Supporting transparency and transformation of IT and telecommunication services B.2 Type of Contract The contract is a Firm Fixed Price. The period of performance may extend beyond the expiration date by up to 6 months, as per Section F.1. Section C: Description/Specifications/Statement of Work Refer to Attachment 1, SOW. Section E - Inspection and Acceptance E.1 Inspection and Acceptance Work is subject to final inspection and acceptance by the authorized Government representative, the Contracting Officer’s Representative (COR). Final acceptance criteria include: Accuracy, clarity, and consistency of work products File editability Format compliance E.2 Approvals by the COR Services will be deemed accepted 30 days after delivery unless the COR provides written disapproval within that period. F.2 Reporting Requirements/Deliverables The contractor shall submit all deliverables as specified in the SOW, ensuring clarity, completeness, and adherence to agreed formats. Draft submissions are allowed for review and comments. The contractor shall proceed promptly and notify the Contracting Officer if directions fall outside scope. G.3 Invoices - Commercial Invoices shall be submitted monthly or quarterly via email to the designated COR and the IPP platform. They must include: Breakout of prices by CLIN Supporting documentation for labor hours and material costs File size limitations apply; backup support should be sent separately if needed Invoices must conform to the payment schedule and other contract terms. All payment inquiries should be directed to the COR. Proprietary data held by the contractor that is unrelated to the operation of the business shall not be subject to delivery under this contract.
