Description

The South Dakota Department of Social Services (DSS), Division of Medical Services (DMS), is seeking proposals for a Case Management Tool to support care coordination for Medicaid members, including juveniles under section 5121, CAA 2023. The tool aims to enhance person-centered services and community-based referrals. This procurement is part of the Medicaid Primary Accountable Care Transformation (PACT) program, which promotes shared accountability for cost, quality, and health outcomes among Medicaid enrolled primary care clinics, Rural Health Clinics, Federally Qualified Health Centers, and Tribal partners. The tool will support care coordinators, case managers, and clinical teams in delivering coordinated services and will be used for up to 90 days of targeted case management and referrals to community-based care and services.
