The Department of Veterans Affairs Salt Lake City Health Care System is seeking Utah-based, licensed mortuary service providers for a four-year Blanket Purchase Agreement to support unclaimed or indigent Veterans. - Government Buyer: - Department of Veterans Affairs (VA) Salt Lake City Health Care System - Network Contracting Office (NCO 19) - Services Requested: - Comprehensive mortuary services including: - Removal of remains (within 1 business day of VA notification) - Preparation, embalming, and body preparation - Storage (refrigeration and secure holding for minimum 14 days) - Cremation (proper urn sealing and labeling) - Casketing and U.S. flag placement - Transportation (professional vehicles, timely delivery to cemeteries) - Burial/inurnment (completion within 5 business days of notice) - Confirmation of interment - Licensing, facility compliance, quality control, confidentiality, and inspection - Quantities & Scope: - Estimated 15-20 cases annually - Annual unit pricing for cremation and burial services (approx. 5 cremations, 15 burials per year) - Delivery to Utah Veterans Cemetery & Memorial Park, Southern Utah National Cemetery, and other eligible cemeteries - Unique Requirements: - Contractor must be Utah-based and fully licensed - Strict adherence to federal, state, and VA policies - Timely response and performance standards (e.g., removal, burial timelines) - Quality control program and confidentiality compliance - Facility must meet NEPA Fire & Life Safety Code and VA accessibility standards - No specific OEMs or product manufacturers are named; procurement is focused on service performance.

Description

The Department of Veterans Affairs (VA) Salt Lake City Health Care System (VASLCHCS) requires a four (4) year Blanket Purchase Agreement (BPA) for mortuary services for deceased unclaimed or indigent Veterans under VA or VA-authorized care. The Contractor shall provide professional mortuary services—including removal, preparation, storage, cremation, embalming, casketing, transportation, and confirmation of interment—for deceased unclaimed or indigent Veterans under VA or VA-authorized care in the state of Utah. Services must support delivery of deceased Veterans from the George E. Wahlen VA Medical Center or any VA-authorized care facility/location in the State of Utah (including contract nursing homes, State Veterans Nursing Homes, and other VA-authorized care locations) to the Utah Veterans Cemetery & Memorial Park at Camp Williams (17111 South Camp Williams Road, Bluffdale, Utah), the Southern Utah National Cemetery (2181 West 1600 South, Cedar City, Utah 84720), or other open state, federal, territory, or tribal Veterans cemeteries in Utah.
