Description

The Minnesota Department of Human Services is seeking proposals for a contractor to provide working group facilitation, project management, and implementation support for Minnesota's 1115 Reentry Demonstration Waiver. This initiative involves multiple agencies including the Department of Human Services, Department of Corrections, counties, Tribal Nations, managed care organizations, and community-based providers. The contractor will serve as a strategic implementation partner to support the transition from planning to execution and establish infrastructure, governance, and operational models for statewide implementation.
