Description

This grant program provides awards to long-term care facilities to positively impact the well-being of residents and workforce, improve operational management capabilities, and build resilience. The program targets Skilled Nursing Facilities, Personal Care Homes, Assisted Living Facilities, and Intermediate Care Facilities that meet specific criteria. Funding supports workforce development and infrastructure enhancements across multiple intervention tracks. The initial period of performance is from January 1, 2027 to September 30, 2027, with potential extensions.
