Provide attestations regarding operator licenses, technologist competency, credentials, health examination records, TB skin test, chest X-ray, hepatitis B immunity, varicella titer, emergency plan, and quality assurance plan. Include make, model, and specifications of the MRI and trailer. Instructions for Past Performance: Provide at least two (2) references (maximum of 5) of similar contracts, including contract numbers, point of contact, phone numbers, magnitude, and period of performance. Instructions for Price: Complete the schedule in SF1449 or provide pricing in your own format (Microsoft or PDF). Quotes must be valid for at least 60 days. Offeror must be certified by SBA as a SDVOSB and registered in SAM, with FAR provision 52.212-3 completed. This solicitation includes FAR provision 52.237-1, Site Visit. A formal site visit is not scheduled. Addendum to FAR provision 52.212-2, Evaluation: The government will evaluate offers using a comparative approach outlined in RFO 12.203(c)(2). The government may consider offers other than the lowest price if they provide additional benefits.
