Description
The Hawai‘i County Office of Aging (HCOA) is requesting proposals from qualified applicants to provide coordination of the Better Choices Better Health (BCBH) – Ke Ola Pono Chronic Disease Self-Management Program (CDSMP). The project aims to continue development and implementation of the BCBH – Ke Ola Pono program throughout the County of Hawai‘i. This evidence-based program, developed by Stanford University, is designed to complement medical treatment and improve disease management by helping participants manage symptoms and communicate effectively with healthcare providers and loved ones. The program is currently offered through HCOA's Healthy Aging Partnerships and includes a targeted Diabetes Self-Management Program.
