The Centers for Disease Control and Prevention (CDC) has submitted information collection requests to the Office of Management and Budget (OMB) for review and approval under the Paperwork Reduction Act of 1995. These collections aim to advance State, Tribal, Local, and Territorial (STLT) governmental agency system performance, capacity, and program delivery.
