Flagler County Health & Human Services is seeking a qualified and licensed provider to fully operate and manage the Adult Day Care Center at the David I. Siegel Center in Palm Coast, FL. - Government Buyer: - Flagler County Board of County Commissioners, Health & Human Services Department, Office of Procurement and Contracts - Scope of Services: - Full operational, programmatic, and financial management of a state-licensed Adult Day Care Center - Facility supports up to 30 participants daily, focusing on adults with cognitive impairments (e.g., Alzheimer's, dementia) - Provider must supply all staffing (minimum 1 director, 5 direct care staff, nursing personnel) - Must deliver therapeutic, social, memory care, clinical health, medication management, nutritional services, facility maintenance, and emergency response coordination - Compliance with Florida Agency for Health Care Administration (AHCA) licensing and Medicaid provider requirements is mandatory - Provider must maintain financial and operational self-sufficiency; no direct operational funding from the County (facility provided at low-cost lease) - Unique/Notable Requirements: - Emphasis on person-centered care and proven experience with similar populations - Strict adherence to state and federal regulations - Vendor must demonstrate financial stability and ability to operate without County funding - No specific OEMs or commercial vendors are named in the solicitation

Flagler County is seeking proposals from qualified, experienced, and licensed providers to operate the County-owned Adult Day Care Center located at 1000 Belle Terre Blvd., Palm Coast, FL. The center has been operating as a state-licensed Adult Day Care facility for 20 years. The selected provider will assume full operational, programmatic, and financial responsibility, ensuring high-quality, person-centered adult day care services with a focus on individuals with cognitive impairments, including Alzheimer's and dementia. Proposals must be submitted via the Flagler County eProcurement portal by August 24, 2026, 6:00 PM.
