The City of Lodi is seeking a qualified service provider to manage and operate the Harmony Homes Permanent Supportive Housing program for homeless residents at 301 East Lodi Ave. - Government Buyer: - City of Lodi Community Development Department, Neighborhood Services Division - Project Overview: - Management and operation of four modular homes (permanent supportive housing units) for up to 12 extremely low-income, homeless individuals - Service provider will determine tenant eligibility, manage property maintenance, provide case management, and ensure compliance with federal, state, and local regulations - Coordination with local social service agencies and the Housing Authority of San Joaquin for tenant-based Housing Choice Vouchers - Products/Services Requested: - Permanent Supportive Housing Operational Management Services (1 contract) - Property management and supportive services, including tenant selection, rent collection, maintenance, and reporting - Interior furnishings for each unit (bedroom, kitchen, living room furniture, and appliances) - Modular unit purchases and installation (5 manufactured tiny homes as part of the Lodi Tiny House Project) - Unique/Notable Requirements: - Provider must have at least three years of experience managing homeless service centers and permanent supportive housing - Must comply with requirements from HEAP, HHAP, PLHA, BCSH, HCD, HUD, HACSJ, and SJCoC - Must operate within a self-sustaining rent-and-subsidy model and maintain property to California Building Standards - Quarterly reporting and data entry into the San Joaquin County Homeless Management Information System (HMIS) - No specific OEMs or product part numbers are named, as this is a service-based RFP with some modular housing and furnishing components

The City of Lodi is seeking a Permanent Supportive Housing Operational Management Service Provider to manage the Harmony Homes project, a permanent supportive housing program for homeless residents in Lodi. The Service Provider must demonstrate experience in permanent supportive housing programs and compliance with federal, state, and local requirements. Responsibilities include determining eligibility for individuals experiencing homelessness, managing property maintenance, providing case management, and ensuring compliance with various housing programs and regulations. The contract term is one year with an option to extend for five additional years based on performance.
