This document announces updates to the Healthcare Common Procedure Coding System (HCPCS) codes on the Master List. It also announces updates to the HCPCS codes on the Required Face-to-Face Encounter and Written Order Prior to Delivery List and the Required Prior Authorization List. The updates include additions of new HCPCS codes based on criteria such as payment thresholds, aberrant billing patterns, and reports identifying high rates of fraud or unnecessary utilization. Implementation of these updates, excluding upper limb orthoses, is effective October 28, 2026, with phased implementation for upper limb orthoses.
