Description
This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Offers are being requested and a separate written solicitation will not be issued. The Department of Veteran Affairs Medical Center, located at 353 North Duffy Rd, Butler, PA 16001 (Butler VAMC), requires a contractor to provide residential placement as part of the Health Care for Homeless Veterans (HCHV) program, in accordance with Veterans Health Administration (VHA) Handbook 1162.09. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. Butler VAMC is seeking to award a five-year Indefinite Delivery / Indefinite Quantity (IDIQ) contract. The contractor must be capable of meeting all requirements in the Performance Work Statement (PWS) in Section B. The contractor facility shall be located within the catchment area of these Pennsylvania counties: Armstrong, Butler, Clarion, Lawrence and Mercer.
Description. This contract will serve for a maximum of 2 veterans per day of immediate placement in a safe environment with onsite supportive services. The daily rate for services is the daily rate per Veteran. The daily rate is all-inclusive to include costs associated with all the services required under this contract. All responsible sources may submit a quotation, bid, or proposal, as appropriate, which will be considered by the agency.
