Statement of Work Purchase of Microscope for the Chief Pathologist Amarillo VA Health Care System Pathology and Laboratory Medicine Services
Statement of Work
The Amarillo Veterans Affairs Health Care System (AVAHCS) Department of Pathology & Laboratory Services (P&LMS) requires new microscope for the Chief Pathologist.
This procurement will update the present equipment inventory to allow the reading of tissue slides to be performed by two Pathologists at one time and forgo the option to share one microscope. The new microscopes will bring added ergonomic and technical advances up to date as well as heighten the quality of images that are required to identify the vast majority of disease states.
The vendor shall provide, in accordance with all applicable federal, state and local regulations, laws, and ordinances, and in accordance with the specifications outlined for accreditation certification if applicable, equipment and supplies as negotiated on GSA contracts to enable AVAHCS to accurately and efficiently identify infectious agents in cells, body fluids, and tissues.
Parts listed on vendor quote are not available in ECAT and will have to be purchased through the vendor.
Vendor Responsibilities The Vendor shall provide a price for two microscopes with 2x, 4x, 10x , 40x, and 100x oil immersion objectives and titling telescopic head. Delivery/Installation
a. The AVAHCS shall prepare the site at its own expense and in accordance with the specifications furnished by the Vendor. b. The Vendor shall incur freight charges and ship the microscope to the address located at: Amarillo Veterans Affairs Health Care System 6010 Amarillo Blvd., West Room 2256-28 Amarillo, TX 79106 Attention: Laboratory c. The performance period shall commence on the date of installation with regards to Maintenance/Warranty Service. Microscope must be compatible with listed parts: See table below.

Training a. Vendor shall provide training for Government personnel on usage of the microscopes.
b. Vendor shall provide on-site training for the use of the microscopes.
Billing
a. Vendor shall send invoice to: Amarillo Veterans Affairs Health Care System 6010 Amarillo Blvd., West Amarillo, TX 79106 Attention: Logistics Service/Maintenance Service/Maintenance shall be covered by a 5-year Vendor warranty at no additional costs.
The Vendor shall provide support assistance with all troubleshooting and repair of the microscope.
Support service shall follow-up all emergency down time calls within one (1) hour and non-emergency down time calls with four (4) hours.
The Vendor shall provide a twenty-four (24) hour/seven (7) day service hotline with technical support.
Vendor shall provide standard and routine hardware upgrades to the equipment without additional charge to the Government.
Standard warranty with options to purchase additional warranty periods.
Licensing and Accreditation The Vendor shall have all licenses, permits, accreditation and certificates required by law and or regulatory compliance.
Personnel assigned by the Vendor to perform these services covered under this contract shall be licensed in a State, Territory, or Commonwealth of the United Sates or the District of Columbia.
