Description

The Hawaii County Office of Aging (HCOA) is seeking information from qualified applicants to coordinate the Better Choices Better Health (BCBH) Ke Ola Pono- Chronic Disease Self-Management Program throughout the County of Hawai'i. The purpose of this Request for Information (RFI) is to gather community input to develop a Request for Proposals (RFP) for the program coordinator. HCOA is interested in strategies for participant recruitment and retention, potential community partners, experience needed for program oversight, and suggestions for future services for older individuals or family caregivers.
