This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation 36C25926Q0676 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through FY26.07B 04/28/2026, FAC 2026-01 03/13/2026. No telephone requests will be accepted. Only written requests received directly from the offeror are acceptable. Background The Oklahoma City VA Health Care System (OKC VAHCS) requires 13 Otoscope/Ophthalmoscope for Primary Care Service Women s Health Clinic.

Objectives/Scope To ensure the safety and comfort of patients due to new facility. The exam will provide new equipment for the OKC VAHCS. The vendor will provide the equipment and required accessories for use at the Oklahoma City VA Health Care System. The vendor will inspect all equipment prior to delivery and installation; any concealed damaged components will be replaced at no cost to the VA. Purchase of American Made supplies remains a priority for the VA.

Deliverables or Deliver Schedule

The contractor shall deliver the units standard delivery with F.O.B. destination with 180 days of the date of the award.
