Description

The proposed action is for a new sole source, firm-fixed priced, contract with option periods to provide brand name or equal to a procurement of a Discharge Care Plan with a twelve month base year and four twelve month option years, ensuring continuity, scalability, and sustained impact on discharge processes across the facility.
