Description
Walworth County is seeking qualified firms to provide licensed Clinical Dietitian services for Lakeland Health Care Center (LHCC). The services include conducting new-resident nutritional assessments, reviewing medical records, medications, laboratory results, and weight trends, participating in monthly weight and wound meetings, and conducting monthly care planning as well as quarterly and annual reassessments.
