Description
This funding opportunity, issued by the Centers for Disease Control and Prevention (CDC) in collaboration with the U.S. Department of Health and Human Services (HHS), aims to implement projects focused on reducing chronic disease rates in children within four specific communities: Lake County, Indiana; Sandusky and Toledo metro area, Ohio; Brownsville, Texas; and Petersburg, Virginia. The project seeks to demonstrate how individual and system-level interventions can encourage healthier habits and choices to reduce the burden of chronic disease in children, with the potential for long-term reductions in adult chronic disease rates. Findings from these demonstration programs will inform replication in additional communities and guide future funding and measurement approaches for health improvement efforts.
