Combined Synopsis-Solicitation for Commercial Products and Commercial Services Effective Date: 01/15/2026 Revision: 03 Description This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format Revolutionary FAR Overhaul (RFO) in Federal Acquisition Regulation (FAR) subpart 12.202, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2026-01. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB). The associated North American Industrial Classification System (NAICS) code for this procurement is 541380, Testing Laboratory and Services, with a small business size standard of 19.0M. The FSC/PSC is Q301, Medical Laboratory Testing. The VA Medical Center, VISN 6 facilities (Fayetteville NC, Durham NC, Salisbury NC, Salem VA, Richmond VA, Hampton VA, Asheville NC) is seeking Legionella Testing Services. All interested companies shall provide quotations for the following: <!-- List of items or services can be added here if specified --> The purchase order/contract period of performance is Base Year from October 1, 2026 to September 30, 2027, plus 4 Option Years. Applicable Provisions and Clauses The following solicitation provisions and clause apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Products and Commercial Services. See Attachment: Instructions to Offerors. All quoters shall submit the following: Offers shall be based on requirements contained in the Performance Work Statement (PWS). Quotes shall include: Offer Form (Standard Form 1449) with Original Signature and Date Contract Administration Data UEI Number Acknowledgement of Solicitation Amendments A completed price/cost schedule and attachments addressing all Contract Line Items Numbers (CLINSs). System for Award Management (SAM) Federal Acquisition Regulations require that federal contractors register in the SAM database at http://www.sam.gov and enter all mandatory information into the system. Award cannot be made until the contractor has registered. Offerors are encouraged to ensure that they are registered in SAM prior to submitting their quotation. All Offerors shall be a verified SDVOSB vendor at the time of quote submission and prior to receiving award. To be considered an SDVOSB, the company must be listed as verified in the VetBiz Vendor Information Pages (VIP) at https://www.vetbiz.va.gov/. Amendments Amendments to this solicitation will be posted on https://sam.gov/. Paper copies of amendments will NOT be mailed individually. By registering to receive notifications on https://sam.gov/ Contract Opportunities, offerors will be notified by email of any new amendments issued. It is the responsibility of the offerors to obtain and acknowledge amendments; failure to do so may result in the offer being considered unacceptable. Quote Preparation Instructions Quote Preparation Costs The Contracting Officer is the only individual authorized to commit the Government to expenditure of funds. The solicitation does not obligate the Government to pay for quote preparation and submission. Quote Format The submission shall be clearly indexed and assembled. Each section shall begin at the top of a page, be numbered, and include the company name, date, and solicitation number in the header/footer. Refer to Table 1 below. Price Submission Submit Original Quote labeled as per Section E.1. Alternate price schedules will not be considered. Total Price will be evaluated including all options. Technical Quote Address all aspects of the PWS. The technical quote should be clear, detailed, and reflect understanding of the work. It shall not exceed 25 pages (minimum 12-point font, ½ inch margins). Exceeding pages will result in only the first 25 pages being considered. Past Performance Information Provide references for similar contracts, including: Company Name and Address Description of Services Performed Responsible individuals' contact info Contract Number Dates of Performance Contract Type and Total Value Include all relevant past performance info as per the award factors. Evaluation and Award Award will be based on a comparative evaluation of quotes per FAR 12. The most advantageous quote will be selected, with technical and past performance factors being more important than price. Submission of a quote signifies acceptance of all solicitation requirements. Evaluation Factors Refer to FAR 52.212-2 for evaluation criteria. Price Evaluation Prices shall be submitted as per the Price/Cost Schedule in Section B. Total price will be evaluated. Options will be evaluated by adding total option prices to the base year. The last year's option prices will be used to estimate six months of effort for evaluation. Additional Information Registration in SAM is required at the time of response submission. Amendments will be posted on https://sam.gov/. Offerors are responsible for obtaining and acknowledging amendments; failure to do so may render the quote unacceptable. Quotes must include a statement regarding acceptance of terms and conditions, either with or without exceptions, with rationale.
