Combined Synopsis/Solicitation Notice Page 14 of 14 Page 1 of Solicitation Number: 36C10X26R0032 Notice Type: Combined Synopsis/Solicitation Synopsis: COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Federal Acquisition Regulation (FAR) 12.2, Solicitation, Evaluation, and Award, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested, and a written solicitation will not be issued. Solicitation number 36C10X26R0032 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Proposal (RFP). This combined synopsis/solicitation is issued under the agency's implementation of the Federal Acquisition Regulation (FAR) Overhaul, as adopted through agency class deviation. All applicable provisions, clauses, definitions, and procedures are those contained in the Overhaul text and the agency's implementation deviations. In the event of a conflict with the codified FAR, the adopted Overhaul text governs. Requirement Description: The Veterans Health Administration (VHA) Equipment Life Cycle Management (ELCM) Program has identified the Electroencephalography (EEG): Electroencephalographic Systems as a candidate for the Department of Veterans Affairs VA-wide (otherwise referred to as national ) Non-Expendable Equipment (NX EQ) contract award. A single award Requirements contract will be awarded IAW all terms, conditions, provisions, specifications, and schedule of this solicitation herein. Offers shall contain the terms for cost/price and technical capabilities of brand name or equal equipment. The Government reserves the right to award without discussions. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 — Electromedical and Electrotherapeutic Apparatus Manufacturing, Product Service Code: 6515 — Medical and Surgical Instruments, Equipment, and Supplies, and the associated size standard is 1,250 employees. There is an existing Non-Manufacturer Rule Class Waiver for Electromedical and Electrotherapeutic Apparatus Manufacturing under NAICS 334510 as of December 20, 2007. This procurement action is issued as a total service-disabled veteran-owned small business set-aside. This acquisition is for a single award Requirements contract with firm-fixed priced (FFP) delivery orders IAW FAR Subpart 16.5 for Natus® brand name or equal NX EQ Electroencephalography (EEG): Electroencephalographic Systems and accessories as identified in ATTACHMENT A — PRICE COST SCHEDULE - NX EQ EEG SYSTEMS. Offerors shall provide a quote for all Contract Line-Item Numbers (CLINs) listed in the attachment. The ordering period is for one 12-month base year with four 12-month option years. Delivery is Free on Board (FOB) destination. FFP Orders will be placed against this contract in writing and will specify delivery locations, dates, and quantities. The FAR provisions at 52.212-1 (Instructions to Offerors-Commercial Products And Commercial Services) and 52.212-2 (Evaluation-Commercial Products and Commercial Services), along with their addenda, apply to this acquisition (see ATTACHMENT D). SCOPE OF WORK The VHA Non-Expendable Equipment National Program has identified the Electroencephalography (EEG): Electroencephalographic Systems product line as a candidate for a VA-wide (otherwise referred to as national ) single award Requirements contract. EEG uses electrodes placed on a patient's scalp to measure, amplify, filter, and record the electrical signals (EEG signals) generated by the brain and subsequently display the acquired patient data in digital wave forms. There are three Neurodiagnostic procedures that Electroencephalographic systems are used for: EEG, Intraoperative Neurophysiological Monitoring (IONM), Long-Term Monitoring (LTM). The first Neurodiagnostic procedure, EEG, is used in areas such as Outpatient Labs, Inpatient Wards, and ICU settings. It observes and diagnoses a range of neurologic conditions, including epilepsy. It can also be employed to evaluate psychiatric disorders and differentiate them from neurologic conditions. IONM has the capability to monitor the functional integrity of neural structures during surgeries such as Carotid Endarterectomy, Corticography, Brain Mapping, Stimulation, Aneurysm Clipping, and the effects of anesthesia on the brain during these procedures. LTM is the collection of EEG data over a period of time. These recordings include Ambulatory EEG (aEEG), which is used for recording at the Veteran's residence. Continuous EEG (cEEG) is used for inpatient video monitoring greater than 2 hours. Prolonged EEG (pEEG) monitoring is used for outpatient video monitoring greater than 2 hours. Video EEG (vEEG) is an elective EEG admission for prolonged video EEG recording performed in an Epilepsy Monitoring Unit (EMU). An EMU is a specialized inpatient unit designed to evaluate, diagnose, and treat seizures. The ability to record and monitor non-invasive scalp electrode EEG, intracranial EEG (Grid and Strip), and invasive stereotactic depth electrodes (sEEG) is the gold standard using state-of-the-art continuous video electroencephalogram (vEEG) monitoring technology. Offerors must demonstrate the ability to meet all requirements for the solicitation. The objective is to provide EEG systems to be used by clinicians throughout VA medical centers and facilities. The period of performance is for a base of 12 months with four 12-month option periods. 2. REQUIREMENT This requirement will be solicited and awarded as Brand Name or Equal, requiring the offeror to indicate that each product being offered is an equal to the Natus® Inc EEG system. The offeror must include a description reflecting the salient characteristics (SC) and level of quality that will satisfy the physical, functional, or performance characteristics specified in the solicitation. The offeror must also clearly identify the item by brand name (if any) and make/model number. Descriptive literature, such as illustrations, drawings, or references to previously furnished data, must be included, along with any planned modifications to conform to the solicitation requirements.
