The State of Alaska Department of Health, Division of Public Health, is seeking a contractor to conduct a comprehensive statewide community health needs assessment. - Government Buyer: - State of Alaska Department of Health, Division of Public Health, Primary Care Office - Contact: Anneliese Zlitni - Scope of Work: - Conduct a statewide assessment to identify communities with unmet healthcare needs, outcome disparities, workforce shortages, poverty, and barriers to accessing care - Collaborate with at least two partners and coordinate regularly with the Primary Care Office Director - Deliverables include a professional, ADA-compliant report for dissemination among stakeholders - Provide a report template for annual updates by the Department of Health - Submit monthly progress reports and attend coordination meetings - Requirements: - Experience in conducting health assessments or similar statewide/regional analyses - Ability to manage large, multi-stakeholder projects and facilitate stakeholder meetings - All services must be performed in the United States - Project budget is firm fixed price, not to exceed $40,000 - No specific OEMs or commercial vendors are named in the solicitation - No goods or hardware are requested; focus is exclusively on research, analysis, and reporting
