Description

Yuba County's Health and Human Services Department (HHSD) is soliciting proposals from qualified, licensed Foster Family Agencies (FFAs) to participate in the Transitional Housing Placement Plus (THP-Plus) Program. The program aims to provide transitional housing and supportive services for former foster youth and former foster parenting youth to help them achieve self-sufficiency and develop life skills prior to leaving the foster care support system. The contract term is two years with two one-year extension options, and the program will not exceed six participant placements per month. Proposals must include details on housing models, program elements, and services such as case management, crisis intervention, therapy access, medical care, educational support, and job readiness training.
