The purpose of this Statement of Work (SOW) is to procure access to a subscription-based, patient-centered discharge care planning platform, specifically the Engineered Care Patient Care Plan (or equal), for use at Overton Brooks VA Medical Center (OBVAMC), 510 E. Stoner Avenue, Shreveport LA 71101.
