Vermont has received a Rural Health Transformation grant of $195 million in Year One of a five-year opportunity to build stronger rural health networks, improve technology and shared operations, and strengthen the rural health workforce. The grant supports facility upgrades in existing rural health care facilities and service delivery sites including hospitals, primary care, specialty care, home health, hospice agencies, skilled nursing, long-term care facilities, adult day centers, mental health providers, substance use disorder providers, and other health care delivery settings. Upgrades aim to support service realignment, shared operations, and integrated care delivery under Vermont's regionalization strategy to ensure long-term overhead and upkeep costs align with patient volume. The goal is to ensure health care services are appropriately sized and distributed across local, regional, and statewide levels, enabling access to necessary care at an affordable price.
