The National Archives and Records Administration (NARA) is proposing to request that the Office of Management and Budget (OMB) renew approval of NA Form 10003, Consent to Make Inquiries and Release of Information and Records, used by the Office of Government Information Services (OGIS). OGIS provides mediation services to resolve disputes related to Freedom of Information Act (FOIA) requests and reviews agency FOIA policies. The form is used to collect written consent from individuals to permit federal agencies to share records and information related to FOIA requests and appeals, as required by the Privacy Act of 1974. Currently, the form cannot be collected online, but OGIS hopes to enable online submission in the future.
