Description
The State of Delaware Department of Health and Social Services, Division of Developmental Disability Services, is seeking professional services to assist individuals in decision-making processes as an alternative to guardianship. The program supports up to 100 individuals per year to develop advance planning directives and other legal decision-making documents to promote choice, autonomy, and access to care. Services include developing Supported Decision-Making agreements, Powers of Attorney, and Advance Health Care Directives. The solicitation is issued pursuant to 29 Del. C. 6981 and 6982.
