The State of Delaware Department of Health and Social Services, Division of Developmental Disability Services (DDDS), is seeking professional services to support individuals receiving DDDS Home and Community-Based Services (HCBS) waivers with advance legal decision-making planning as alternatives to guardianship. - Government Buyer: - State of Delaware Department of Health and Social Services - Division of Developmental Disability Services (DDDS) - Contracts, Management and Procurement Office - Services Requested: - Professional services to assist up to 100 individuals per year - Development of Supported Decision-Making agreements, Powers of Attorney, and Advance Health Care Directives - Assistance with uncontested guardianship terminations where appropriate - Outreach and community education on alternatives to guardianship - Requirements: - Vendors must provide licensed paralegal/advocate and attorney supervision - Subcontracting is not allowed - Compliance with all applicable state and federal laws - Vendors must hold appropriate Delaware licenses and certifications - No storage or transfer of non-public State of Delaware data outside the United States - Monthly usage and diversity reporting required - Payments are milestone-based, not in advance - No specific OEMs or product vendors are named in the solicitation - The contract is for one year with four one-year optional extensions, and may be extended month-to-month for up to three months after the full term - Pricing must remain fixed through June 30, 2028

The State of Delaware Department of Health and Social Services, Division of Developmental Disability Services, is seeking professional services to assist individuals in decision-making processes as an alternative to guardianship. The program supports up to 100 individuals per year to develop advance planning directives and other legal decision-making documents to promote choice, autonomy, and access to care. Services include developing Supported Decision-Making agreements, Powers of Attorney, and Advance Health Care Directives. The solicitation is issued pursuant to 29 Del. C. 6981 and 6982.
