Description

The City of Sierra Vista is seeking proposals from qualified medical providers to conduct physical examinations for prospective and current employees who require a valid Commercial Drivers' License (CDL) certification. Services include post job offer/pre-employment physicals, occupational health medical services, and Arizona Department of Transportation CDL examinations. The contractor must be able to perform screenings, immunizations, and vaccinations at medical offices within Cochise County. The contract term is two years with an option to renew for three additional one-year periods, not exceeding five years. The City does not guarantee the quantity of persons requiring services, as it is on an as-needed basis.
