Description
The Department of Veterans Affairs, Network Contracting Office (NCO) 6, intends to enter into a sole source contract with AGILITI HEALTH, INC. This requirement is for rentals of mobile laser machines assisted by licensed and credentialed technicians. Description of service/supplies being requested: Argon Beam Coagulator, Argon Beam Coagulator Stand By, CO2 laser, CO2 laser Stand By, Holmium Laser Low Wattage, Holmium Laser Low Wattage Stand By, Aura XP Laser, Aura XP Laser Stand By, Aura Accustat Fiber, KTP Laser, KTP Laser Stand by, OmniGuide Laser, OmniGuide Laser Stand By, UltraPulse CO2 Laser, UltraPulse CO2 Laser Stand By, XPS Laser, XPS Laser Stand By, Ultrasound. SALIENT CHARACTERISTICS General Requirements (All Equipment) All systems provided shall: Be commercial, mobile, and suitable for hospital Operating Room environments. Arrive fully functional, calibrated, and ready for immediate use. Include all necessary accessories, cables, disposables, and interface components required for operation. Be compatible with the facility's standard electrical supply and OR workflows. Include a trained technician capable of setup, operation support, and troubleshooting. Include access to a backup unit when required to avoid case cancellation or delays. Surgical Laser Systems - Minimum Functional Requirements The contractor shall provide laser systems meeting the following minimum capabilities across the full range of requested surgical modalities (CO, Holmium, KTP, Argon Beam, etc.): CO Laser Systems Capable of performing ablative, coagulative, and cutting functions. Provide adjustable power output suitable for ENT, dermatologic, gynecologic, and urologic procedures. Support both focused beam and fiber delivered energy delivery (where applicable). Include a range of handpieces, adapters, or fiber delivery options sufficient for common procedures. Deliver a spot size suitable for precision surgical applications (e.g., fine cutting ENT work). Holmium (Standard & High Power) Capable of lithotripsy, soft tissue ablation, and urologic surgical interventions. Deliver fiber based laser energy via holmium compatible fibers. Variable power settings suitable for both low and high power applications. KTP & Greenlight Type Lasers Capable of photoselective vaporization and coagulation for urologic and ENT procedures. Support delivery of energy through standard disposable or reusable fibers. Argon Beam Coagulator Provide non contact argon plasma coagulation for open surgical procedures. Deliver precise, controllable coagulation for tissue desiccation. Vbeam / Pulsed Dye Laser (if requested) Provide pulsed energy delivery for dermatologic vascular applications. Adjustable pulse width and energy settings appropriate for clinical use. Cryoablation System - Minimum Functional Requirements Provide percutaneous or intraoperative cryotherapy using argon based or equivalent technology. Support manufacturer approved cryoprobes appropriate for urologic cryoablation. Include temperature monitoring and control features. Provide freeze thaw cycles capable of ablative tissue destruction. Be mobile and operable in a standard OR suite. Fusion Biopsy System - Minimum Functional Requirements Support real time fusion of pre operative MRI images with live ultrasound imaging. Be compatible with the contractor provided ultrasound system (e.g., BK5000 type capability). Include biopsy guides, grids, or stabilizers appropriate for targeted prostate biopsy. Support acquisition, alignment, and guidance for targeted tissue sampling. Provide workstation/software capable of image fusion, contouring, and biopsy path visualization. Ultrasound System (BK Type) - Minimum Functional Requirements Provide real time imaging suitable for urologic and surgical applications. Include transducers/probes appropriate for fusion biopsy and intraoperative imaging. Include system specific disposable items such as biopsy guides, if required. Provide image quality adequate for lesion targeting, guidance, and intraoperative visualization. Technician Requirements (As Salient Characteristics) Technicians supplied must: Have a minimum of 2 years experience operating the equipment assigned. Be manufacturer trained and certified on each modality supported. Be trained in laser safety and applicable clinical safety policies. Arrive at least 30 minutes before the procedure or earlier if required. Be available within 30 minutes for emergency or after hours support. Be capable of performing setup, safe operation, and troubleshooting. Safety & Compliance Requirements The equipment and technicians must comply with: All applicable FDA classifications for medical devices. OR laser safety standards and facility policies. Infection control and cleaning requirements. HIPAA and privacy protections (for any recorded patient data). Contractor responsibility for maintaining equipment in safe working order. Accessories & Consumables For each procedure, the contractor shall provide: All required fibers, handpieces, probes, grids, and guides Single use or reusable accessories required by manufacturer instructions Backup consumables as needed to prevent cancellations Performance Requirements Equipment must remain operational for the duration of the procedure. If equipment failure causes a delay > 60 minutes, the VA will not be billed. Backup units must be available when required to prevent procedure cancellation. Contractor shall respond promptly to intraoperative malfunctions. STATEMENT OF WORK Scope The contractor shall provide commercial per procedure rental services for mobile surgical lasers, cryoablation systems, fusion biopsy systems, and ultrasound systems, including delivery, setup, operation support, removal, and manufacturer trained technician services at the Richmond VA Medical Center, 1201 Broad Rock Blvd, Richmond, VA 23249. The Government will schedule procedures requiring these systems and will pay on a per procedure basis according to the pricing provided in the contract. Equipment and Service Requirements Surgical Laser Systems Includes multiple modalities used in Urology, ENT, Gynecology, Dermatology, and General Surgery, such as: CO surgical lasers CO cosmetic lasers CO fiber optic systems CO AcuPulse Duo systems CO Omniguide systems Argon Beam Coagulator Holmium systems (standard and high power) Greenlight XPS KTP lasers (standard and Aura) Vbeam lasers All systems provided shall: Be mobile, OR portable systems suitable for hospital surgical use Fully operational at delivery Equipped with required accessories, adapters, handpieces, fibers, and disposables Supported by a backup unit if the primary unit becomes unavailable Cryoablation Systems Provide a mobile cryoablation platform, including: Cryo console Required probes Consumables and accessories Setup and intra operative technician support Fusion Biopsy Systems Provide a mobile fusion biopsy system capable of: Real time MRI/Ultrasound fusion Use with BK ultrasound equipment Required biopsy grids, needles, guides, and software Technician support for setup, calibration, and intra operative assistance Ultrasound Systems Provide BK5000 ultrasound systems and associated probes, guides, and consumables for procedures requiring real time imaging support. Technician Requirements The contractor shall provide a qualified, manufacturer trained technician for each procedure requiring the equipment. Technician duties include: Delivery of requested system(s) to the Operating Room Pre procedure setup and diagnostics Intra operative support as required Troubleshooting and resolving performance issues Removal of equipment after procedure completion Technician qualifications: Minimum 2 years of experience operating assigned equipment Manufacturer specific training and certification Laser safety training Availability to arrive at least 30 minutes prior to procedure start, or earlier if required Ability to respond to after hours or emergency procedures within 30 minutes Delivery and Performance Requirements 4.1 Delivery The contractor shall deliver equipment to the specified OR suite on the scheduled day of service. 4.2 Equipment Condition All equipment delivered shall: Arrive fully operational Have passed contractor maintenance and safety checks Be clean and ready for surgical use 4.3 Operational Failure Clause If equipment is inoperable for 60 minutes or more, causing the procedure to be postponed, the contractor shall not charge the Government for the affected procedure/day. Scheduling and Cancellations 5.1 Scheduling The VA will provide at least 24 hours notice for scheduled procedures when feasible. Emergency cases may require shorter notice; technicians shall be available within 30 minutes as required. 5.2 Cancellations Either party may cancel a scheduled day with 24 hours notice. For cancellations within 24 hours of start time, the contract's cancellation fee (if applicable) is governed by the pricing schedule. Government Responsibilities Ensure surgeons are trained and competent in the use of supplied systems. Provide a safe and adequate facility for equipment operation. Handle all clinical care, patient monitoring, and medical decision making. Contractor Responsibilities Maintain and service all provided equipment Provide all required disposables, fibers, and accessories for each procedure Ensure technician compliance with VA privacy, infection control, safety, and conduct policies Maintain appropriate liability and equipment insurance Ensure zero unauthorized PHI disclosures Maintain personnel background screenings as required Place of Performance Richmond VA Medical Center 1201 Broad Rock Blvd Richmond, VA 23249 Period of Performance One (1) Base Year as aligned with your acquisition plan for a single-year award. Invoicing Invoices shall be submitted monthly and include: Date of service Procedure type Equipment used Number of hours (if applicable) Any add on or emergency fees Only performed procedures may be invoiced; postponed procedures due to equipment downtime may not be billed. Period of performance will be for the period 06-23-2026 through 06-22-2027. The Government intends to solicit and award with one source under the authority of Revolutionary FAR Overhaul (RFO) 12.102 (a). This notice of intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be accepted. However, any business that believes it can meet these requirements and possesses relevant past experience and expertise may give written notice. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirement. Responses will be considered by the Government, however, a determination by the Government not to compete the proposed acquisition based upon responses of this notice is solely within the discretion of the Government.
